Job Description LanceSoft is seeking a travel nurse RN Care Manager for a travel nursing job in Lebanon, New Hampshire. Job Description & Requirements - Specialty: Care Manager - Discipline: RN - Start Date: 04/27/2026 - Duration: 13 weeks - 37.5 hours per week - Shift: 8 hours, days, on call, rotating - Employment Type: Travel 8-4:40 Monday - Friday No weekends or on call Must be a certified care manager BSN 3 years inpatient clinical experience At least 1 year Care Management experience The RN Care Manager will oversee a daily caseload of patients on the inpatient units with a variety of care needs. Will attend interdisciplinary rounds. Complete Initial assessments. Facilitates the development of a plan of care in collaboration with the patient, family, caregiver and health care team. Communicate consistently with medical teams and patient regarding progress of care needs related to post discharge care needs, such as: home health, inpatient rehab. Will interact with post acute care referral sources to establish a plan. Document and Place orders for home health, DME, nutritional needs within EPIC EMR.
