About This PediPal Medical Social Worker PositionHopeHealth is seeking a dynamic and motivated Pediatric Social Worker for our Pediatric Palliative Program. The Clinical Social Worker is responsible for planning, implementing, and evaluating social work services to the patient in his/her place of residence.This is a part-time, 24 hours per week position with flexible hours. Some on call time for phone calls. This position is eligible for benefits. The Pediatric Palliative Care Network (PPCN) is designed to meet the needs of children with life limiting illnesses and their families in the Plymouth, Norfolk and Bristol County service areas. The Pediatric Palliative Care Social Worker will be knowledgeable about the PPCN program and its operation. The MSW will conduct psychosocial visits with families, some telehealth based. The Social Worker will be a resource to staff and other referral sources. What will you do?Responsible for the coordination and delivery of palliative care services to children, adolescents, and their families.Educates patient, family and staff on diagnosis, treatment programs and resources availableConduct psychosocial, emotional, and socioeconomic assessment of children with life threatening diseases and their families Collaborate on care plans with evaluation of patient outcomesCollaborates with other members of the Pediatric Palliative Care team to assure high quality care and assure coordination of services Adhere to the organization's confidentiality policy regarding patient informationCollaboration with bereavement team for grief counseling services Documentation of all contacts and patient activity in EMR Participates in special events for familiesUnderstand the dying process and manage the emotional impact of working with the dying and bereavedCommunicate regularly with the Pedi Pal Program Manager to feedback progress or concerns regarding any aspect of clinical work and to structure workload appropriately Participate in weekly IDT meetings and report on managing the goals of care for patient and familyClinical expertise and competency in the practice of standard precautions and Agency infection control practicesHelp bring quality of life to our Pedi-Pal PatientsHave a strong understanding of conditions, causes and treatments of a variety of disordersVerbal and written communication skills needed to communicate clearly, effectively, and patiently on the phone and in email/text messagesProficiency with computers, PowerPoint, Zoom and healthcare databases.What are the qualifications for this job?Master's degree from a school of social work accredited by the Council of Social Work.Experience working with children preferredCurrent licensure in the state of Rhode Island and Massachusetts preferred.Two or more years of social work experience relevant to health care.Experience with community resources helpful.Demonstrated skills in communication, documentation, assessment and social work methods.Reliable, independent means of transportation to make home visits. Valid driver's license and automobile insurance.
